Pre-Installation Hyperbaric Medicine Survey

Thank you for your interest in developing a Hyperbaric Medicine Program. This survey will help us determine the potential patient base in your service area. We have broken the survey into inpatients and outpatients for a two year period of time. If you could have your Information Systems Department check back as many ICD-9 codes as possible, we will get a better picture. As you are probably aware, hyperbaric and wound care codes are not necessarily used for principal billing codes. They are often the second, third or even greater code listed.
